SHARKS BASKETBALL CAMP
4 DAYS
Monday 20th – Thursday 23rd January 2020
All levels welcome (beginners, intermediate, advanced). Participants will be divided into classes (age and ability).

Children are expected to attend all four days.
ENROLMENT AND PAYMENT MUST BE COMPLETED BY Monday 16TH December 2019
PARENT/GUARDIAN CONSENT: By submitting this form, I approve of this enrolment and understand that the organisers will take all reasonable care to ensure the well-being of my child/children during the course of the camp. I will not hold them responsible for any injuries that occur during the camp. I give my consent for any necessary medical treatment and agree to meet any and all expenses incurred. I authorise that any photographic or video images taken at the camp may be used for promotional use.
I understand that if my child’s behavior is inappropriate, I may be asked to pick him/her up.
